Whether you're a support coordinator, GP, family member, or participant self-referring — our referral process is simple, fast, and person-centred.

How It Works

Simple Steps
to Get Started

Referring someone to Live Well is quick and straightforward. Complete the form and we'll take it from there.

Submit the referral form with as much information as you have. You don't need to have everything — we'll follow up to gather any missing details.

We review and respond within 1–2 business days to confirm receipt and discuss the referral with you.

We contact the participant (or their guardian) to arrange an intake conversation and understand their goals and needs.

A personalised support plan is created and support commences — usually within days of completing the intake.

Who Can Refer?

We welcome referrals from:

  • NDIS participants (self-referral)
  • Family members and carers
  • Support coordinators and plan managers
  • GPs and allied health professionals
  • Hospitals and discharge teams
  • Other disability or aged care services
